Suggest Treatment For Nystagmus To The Right Eye

Question: Hi Doctor:

I have been having balance problems mostly while walking feeling like I am swaying to one side or walking on a ship.Went to neurologist and she said I have a nystagmus to the right eye which may or may not be causing the balance problems. She ordered an MRI of the brain and auditory canal. I had a meningeoma 22 years ago, successfully removed; had right sided paralysis which resolved. I do have right foot drop as a result of the surgery. had a CT of the brain about 2 months ago which showed encephalomalacia changes but everything else good. Last MRI of brain was about 6 years ago. Not sure what the nystagmus means and what the neuro is looking forr? Thank you for your help.

Hello
Wishing you an early and complete recovery.
Giddiness and swaying to one side may remain as a persistent symptom of post operative intracranial lesion( meningioma ) in some patients .
Mostly it is secondary to Gliosis (encephalomalacia )which remains as a scar in brain , and that part of brain is actually not active neurologically.
But if repeat scan 2 months back is showing NO RESIDUAL TUMOR , it,s good .
Otherwise also MENINGIOMAS are very rare to recur.
For giddiness you can continue with medications like BETAHISTINE / STEMETIL etc.
If it still persists you can have a CARDIOLOGY/ ENT check up to look for other causes of giddiness .
NYSTAGMUS is involuntary repetitive horizontal/vertical movements of eye balls as a sign of incoordination .
NEURO is looking for encephalomalacia / recurrence of lesion by repeating CT/MRI scan of brain .

Is it a common occurrence to have encephalomalacia. I had my craniotomy 22 years ago, but whenever I go for scans the only thing that comes up is encephalomalacia changes everything else is good. White and gray matter are fine. Could this be a problem now that I am 62 years old? Thank you again for your help.

Hello
I have gone through your follow up querries in detail.
As such eye nystagmus is not a disease , it,s just a sign of brain disease/ encephalomalacia.
There is no treatment needed for nystagmus as such .treatment of imbalance is with BETAHISTINE/CINNARIZINE /stemetil etc.
Encephalomalacia is a common occurrence after tumor excision.it,s a brain scar .and it remains for life . If your scans are ok after 22years after craniotomy ,it means there is no reccurrence .
Continue with symptomatic treatment .
