Question: Hi Dr, I am 32 year old. Weigh 120kg. Been on heavy. side for many years. I have never had high sugar levels and it was always way below the 110. Recently last 4 months my job is extremely stressful and long hours. My eating has been very poor and untimely eating like at 12 in night etc. Now I see my fasting sugar level at 133. I don't pass urine many times like I read is one of symptoms for diabetes. Can you please let me know if it's serious. Can I reverse it completely. I am really worried. I have started going for jogging and walking. My call my does have history of sugar but not at 32age

Thanks for your question.
Noted your concern.

Being obese (Possible BMI more than 40, you have not mentioned your height), family history of diabetes and stress full lifestyle you are at high risk for progression to Type 2 diabetes mellitus.

Your fasting sugar level of 133 if done in laboratory its self confirm the diagnosis of diabetes. If it is done with glucometer I may suggest -
- OGTT (Oral Glucose Tolerance Test).
- HbA1c

These test results will help in confirmation of diagnosis of diabetes.

Most of the patients wont have osmotic symptoms (abnormal thirst, increased frequency of urination, weight loss) at the time of diagnosis of diabetes. And detected as having diabetes during routine health check ups or evaluation for some other problem.

You should start working on weight reduction.
- Diet modification: Calories and fat restricted diet.
Better to consult near by dietitian who can plan diet for you considering your BMI and food habits.
- regular Exercise.

Also following tests may be advisable in case of obesity-
- Lipid Profile.
- Ultra TSH.
- calcium Profile.
- Renal function test.
- Liver function test.
- Vitamin D3.
- Urine examination.

You should consult diabetologist or endocrinologist for detailed physical examination to consider any pathological causes for obesity.

But Dr 6 months back it was Normal only at 100 something on fasting. Is it fully reversible Dr. Please suggest. I will consult and do necessary exercise etc

Insulin resistance is a progressive.

Detailed Answer:
Type 2 diabetes is due to presence of insulin resistance it means there is resistant to action of insulin and body requires extra insulin to maintain normal metabolism and blood sugar level.
In the initial part of life our pancreas is capable of producing extra insulin as per body requirement but as age of person advances pancreas cannot cope with need for extra insulin and blood sugar levels start rising as in your case.

Insulin resistance can be reduced by significant weight reduction.
So confirm your diabetic status by appropriate blood tests.
And whether diabetic or not work on weight reduction under supervision of diabetologist (or endocrinologist) and dietitian.
In really resistant cases bariatric surgery may be advisable.

You should follow standard method.

2 hours post-prandial blood sugar may be used as substitute to 2 hours post Glucose load blood sugar level in Oral Glucose Tolerance Test (OGTT) in clinical practice.
But it is not standardized because meal content may vary from person to person.
The standard method used to define diabetes or pre-diabetes are -
- OGTT.
- HbA1c.

So I may suggest to do HbA1c which dose not require any preparation and done on a single blood sample which does not require Fasting or any particular timing of sample collection.
Which tell better information.

If we considered 2 hours post-prandial blood sugar for 2 hours post Glucose load blood sugar level in OGTT still it denotes Impaired Glucose Tolerance (Prediabetes).
