I am not sure I understand you question - needs clarification.
You are a 28 year old woman with a history of kidney disease and now you have fever an pain?
Here are some questions that will help me to answer better:
1. When did the fever and pain start?
2. How high is the fever?
3. Is the pain in the kidney area (in the back at the side, behind the waist)?
4. What kidney disease has she had? Kidney stones? Infections? Something else?

Brief Answer:
Kidney infection (pyelonephritis)
Detailed Answer:
Fever plus pain in the kidney area is likely to be a kidney infection (pyelonephritis). She needs antibiotics, most likely via IV. Kidney infections are serious and need to be treated quickly. So please take her in to the ER. There they will get a urine specimen to check for bacterial infection, draw her blood for a complete blood count, and may possibly also get an X-ray. Make sure that the urine is sent for a "culture and sensitivity" test. This test finds what bacteria is causing the infection, and which antibiotics are most effective for it.
After this they will give her antibiotics and she will start to feel better soon. The antibiotics they give will not be based on the "culture and sensitivity test" which takes a couple of days. So, if after the test results, if she is not getting better, they will use a different antibiotic based on that test.
So... please take her in to the ER now for treatment of what is likely to be a kidney infection.
