Brief Answer:
Worthwhile to consult surgeon / urologist...

Detailed Answer:
Hi,

Both testis are not identical. Almost all men have unidentical testicles and one will always appear larger and lower than other. However if you experience pain along with size difference, it should be evaluated. Possibilities of torsion, infection / inflammation always exists. A good physical examination and if needed an ultrasound scan should be performed to identify the cause.

Pain from testicular region at times radiates to kidney area. Therefore it will be worthwhile to meet a surgeon or urologist and get evaluated.

Hope this helps.

Regards
