Brief Answer:
Yes it can recur

Detailed Answer:
Hi I am Dr Gopal Goel , Orthopedic Surgeon.
You had carpal Tunnel surgery many years ago and are having symptoms suggestive of carpal tunnel syndrome now again.
Carpal Tunnel compression can occur again many years after surgical release .
So I would suggest you get a nerve conduction velocity test to confirm the diagnosis. If the diagnosis is confirmed then re surgery may be required.
I hope to have answered your question. Please feel free to ask any further questions.

Brief Answer:
Take pain killers and pregabalin

Detailed Answer:
Hi I am Dr Gopal Goel Orthopedic Surgeon
You can try pain killers and Pregabalin and a night splint (to keep the wrist straight) to relieve you of pain at night.
But if the compression is more than these may not help.
I hope I have answered your question.
