Question: Young man of 19 years old on his feet walking for hours a day. Complains of pain on the bottom of the large toe joint. States his shoes are a bit worn out. He sees no redness, swelling, or callus. Time for new shoes or additional support in the toe of shoes?

I think shoe change with silicone insole help

Hello, I have studied your case. Pain and swelling at bottom of toe is suggesting that there can be mismatch between shoe and feet. I would suggest you following :

1. Change shoe. Try to take soft and loose one. Also apply silicone insole inside.
2. Hot water fomentation
3.steroid injection at base of toe if their is any pain.
4. Avoid bare foot walking.
