Question: I have a history of Gout. Just recently, two weeks ago I had a attack. I get affected in my ankles. Just the other day I felt something similar to that pain/tightness in my right knee. Started in lower inside right of my knee, now I have tough time straightening out the knee. Is this Gout as well?

Brief Answer:
Probably not if you do not have redness and local heat sensation

Detailed Answer:
Hello,
Thank you for asking at HCM.

I carefully read your question and understand your concern.

Gout is a disease that is related to some cristals deposits in the joints. it mostly affects the bit toe and ankles but might affect the knee also.

It generally causes redness, swelling, pain and local hit on the joint area and it is very tender to move the joint- probably these are the symptoms you are already having on the ankle.

If you have similar symptoms on the knee: redness, swelling, local heat, other than pain and tightness feeling than it is very likely that you are having a gout attack on the knee also.
If you do not develop the above symptoms on the knee it is very unlikely to be gout.

Something else that might happen is that when we have pain in one area or in one joint we tent to overuse other areas or joints to protect the one that is hurting. This in itself might bring pain or discomfort in other joints also. The knee is a joint that is commonly affected when we have pain on the ankles.

1. I would recommend you to take Advil regularly in high doses- high doses are anti-inflammatory while smaller doses than 600 mg a dose are generally just pain killers and will not help with swelling and redness. So take 800 mg of advil after food every 6 hours for next 2-3 days. Afterwards, you can take Tylenol (ACETAMINOPHEN) 650mg thrice daily after food.

2. Pain killers can hurt the stomach so if any stomach pain please seek medical help soon. prophylactically you can also take PPI/antacid eg. PANTOPRAZOLE 40 mg once daily before breakfast for the time being.

3. RICE- rest/ice/kill pain and/Elevation are helpful

4. Even if you are having gout in that knee still the treatment would be the same as above. The above treatment would help even if the pain is due to overuse of the knee because of the hurting ankle

5. Sometimes because of repeated episodes of gout it is recommended to check uric acid levels in blood and if they are high, it is recommended that while the episodes subside to start Allopurinol to prevent the future gout episodes (prescription drug, so only after consulting your doctor)

6. If no improvement with Advil, Tylenol and RICE please see a doctor.

I hope this is helpful and answers to your question.

Kind Regards,

Dr Antoneta Zotaj
