Question : For several years I would get occasional pain in my clatoris. It would pass. This past week I started having severe episodes of this pain that starts out with a sharp pain that increases in waves until it climaxes. It is severe. I scream so bad that my husband wants to take me to the hospital. When it is over I am back to normal. But it terrifies me to go anywhere because I'm afraid it will happen in public. I am 58. A woman. And have Secondary progressive multiple sclerosis. I don't even know what kind of doctor I should see. I told my neurologist about it. But when I brought it up, she seemed to ignore my question. When I told her the pain was not this severe. I went to my G. P. I told him what was happening he seemed to think because it wasn't happening right then that he didn't see the need for examining the area. Please help me with this. I feel very depressed. Anything would be appreciated.

Brief Answer:
Likely to be vulvodnia.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
From your description it seems that you are suffering from vulvodynia.
It is a difficult problem to treat and only some symptomatic relief is seen with the help of medicines.
In addition to maintaining a good hygiene, you need to avoid activities that put direct pressure on the clitoris. This includes bicycling and horseback riding.
Sit on a foam rubber doughnut.
You can also do cold sitz bath for pain relief. Doing relaxation exercises will be helpful.
Medical treatment includes topical anaesthetics, estrogen creams, nerve blocks, tricyclic antidepressants, gabapentin, pregabalin etc. You need to discuss these options with your gynecologist for relief.

Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.
