Brief Answer:
Hormones

Detailed Answer:
Sorry to learn about your symptoms.

1 Testosterone of 295 is not severely low

But when I see someone like you in my practice, I investigate this level systematically as per current guidelines. This includes

a thorough physical examination

repeat check of blood Testosterone levels before 9 am

along with testosterone the following tests are also ordered by me:

CBC
CMP
TSH
Free T4
Prolactin
FSH
LH
Ferritin
SHBG
Free Testosterone (by a special method called Equilibrium dialysis)
25 hydroxy vitamin D

2 Only once it is amply clear that one has testosterone deficiency would I proceed to offering treatment options.

3 Now that you are already on testosterone, running these tests will not give a clear answer. So only some of these can be done now; others can be done only after you have been off testosterone for several months
CBC
CMP
Prolactin
TSH
Free T4
25 hydroxy vitamin D

4 I have noted you other medical history. I would recommend going about evaluating you in this manner as explained above
