Question: extreme "no cause" pain in back left bottom shoulder blade....been 24 hours..400mg ibprophein and traumel have not relieved any of the pain

Hello,

I know that your main concern must be to relieve the pain, but a pain of this intensity definitely needs investigation. Does it relate to the movements of your body or arm? Does it relate to the respiratory movements?

Potential causes may include:
- various orthopedic causes (like muscle sprain)
- herpes zoster even if no skin lesions are visible
- lung problems (like pulmonary embolism, pneumonia, pleuritis etc)
- heart and blood vessel problems (like myocardial infarction, pericarditis, etc)

Regarding pain relief, you may add paracetamol (acetaminophen) to the regimen and perhaps use more tramadol but the investigation is more important.

Follow up: Dr. Panagiotis Zografakis (16 minutes later)
It is a constant pain (like severe pressure of a ball pin hammer)but sharpens to extreme with certain movements of head/neck (to the left) and arm movement. I am getting some cracking/creaking noises with movement as well when I try and rotate the cuff. No pain with breathing. I am fairly confident I don't have any variation of herpes....

The location of the pain and its relation to movement points to an orthopedic cause. If it were lower, it could have been the kidney (renal colic). Anyway, investigation is needed. Until then, do not take any more ibuprofen. Try some paracetamol and if it's not helping, you can use some more tramadol. Beware of the nausea the opioids may cause.
