Suggest Treatment For Pain In Shoulders, Neck, Hands And Legs

Question: My husband has since March had pain in his shoulders neck hands and legs. Some days the pain is not too bad other days it's chronic. He's been to hospital with a very rapid heartbeat but every test he's had has been negative. The only part of his body they haven't checked is hi head. Blood tests were all normal.

Well, the sort of pain you are describing which has not been picked up on blood test or X rays is a bit challenging then.

I would like to ask you a few more questions before guiding you further:

1. Does he have swelling as well..?

2. Was his Vitamin D 3 levels done?

3. Is it more in the mornings or night?

4. What about the ESR?

5. Any painful movements in the shoulders?

He has swelling in his left hand
Vitamin D levels are normal
His shoulders are very painful and worse with movement. He also has weakness in both arms.
Pain is worse in the mornings

Well, the sort of pain you are describing does fit in with inflammatory pain considering it is worse in the mornings. I am not sure if his ESR has been checked yet as these sort of pains are common in Polymyalgia Rheumatica. It can even present with arthritis considering the swelling in the hand.

2. I would like to know if his shoulder movements are ok. My next differential would be to check for his movements if he is able to take the hands behind his back or keep it behind his head or lift it up. If those are normal, at least we know the joints are ok.

3. My last differential would be cervical spondylosis compressing on his nerves causing the weakness. If that is the case, he will need early intervention either with traction or surgery

Could you check these movements on him and also his ESR and CRP report. Also let me know what the doctor thinks about it. SO i can guide you further

Thankyou Doctor. Our GP prescribed anti inflammatory drugs which help a little but only mask the problem.
He's been tested for rheumatoid arthritis several times but all negative
Same for spondylitis. ESR normal
All tests so far are normal.
My husband has been fit and active all his life does not smoke rarely drinks and is not overweight.
The doctor said its just old age !
He has some pain when putting his arms behind his back and head

Periarthritis of the shoulder

Well it may fit in well with periarthritis of the shoulder, that is inflammation around the shoulder capsule which can happen with age. Also it is common in Diabetic patients.

At this stage, I would suggest you go for Physiotheraphy considering he has only partial response to anti inflammatory drugs. If it still doesnt improve it is worth while considering intra articular injection to release the capsule and let the inflammation subside. Injection if given well, work like magic and he can get back to his usual self.

My final impression would be Peri arthritis of the shoulder( Probably Degenerative)

Thankyou Doctor it certainly is a mystery. Could he have arthritis even if all the tests for that are negative.

My daughter was talking to her girlfriend about XXXXXXX and her father had similar and he was diagnosed with a pituitary gland tumour. Could this be possible

Arthritis due to tumour

Well the arthritis can be 2 types...One which is inflammatory( looks less likely in your husband as all the tests have come out negative). Second is degenerative( age related, cartilage between the bones gets deficient which looks more likely in his case. IT has to be managed with physiotherapy and injections into the joints as and when needed

But Arthritis due to tumor, is a very unlikely thing to happen. It can present but then you have more than just the shoulder joints. It is always good to get a Chest X ray done as well as shoulder joints can be involved with it. But if there is a tumor one tends to have certain symptoms of it like fever/weight loss/appetite loss and other symptoms depending on the tumor. It does seem a bit crude to get whole body SCANS done without any symptoms. I wont go too much in favor of it as something would have appeared in blood by now if it was a cancer of any sorts

So , dont worry about it. We need to get his shoulders fit and fine.

Thanks doctor. I wouldn't keep following this through with you but my husbands condition is getting worse.
He has a 40 minute drive each way to work and he can barely turn the steering wheel.
He now has chronic pain in the evenings.
Other symptoms include tiredness and irritability and he is really feeling the cold this year which he hasn't felt in the past.

He used to stay up late every night and tonight he's asleep on the lounge and it's only 6.45pm

He said earlier tonight that his pain is a lot worse.

He had physiotherapy on Wednesday (two days ago) which helped for about 12 hours and then the pain returned.

Increasing fatigue

Wel the sort of fatigue he is having is a bit unusual and something one should not ignore, especially at his age. I feel there is something more going on and he needs to be evaluated thoroughly. Considering the decline in his general functions, one needs to figure out the cause

I would suggest you consult a different GP and let him have a fresh look so they dont get biased by the previous diagnosis. If it is only shoulder pain, Intra articular injection would work. But we need to sort out the reason for fatigue as well

Also supplement him with Vitamin D tablets , which would help combat the fatigue
