                                    Brief Answer:
He may need a scrotal scan, if severe pain.
Detailed Answer:
Hello XXXX and welcome to HCM.
As an Urologist, i can fully understand your concern.
If the pain and swelling over the testis is severe, the safest step is to get him examined, by an Urologist.
If needed, the doctor usually gets an ultrasound scan with doppler of the scrotum.
That will show very clearly, the extent of the injury, if present.
If not severe, just a scrotal support is enough, and review in two days.
If severe, treatment depends on the report.
The knot on the left testis can be made out very clearly.
Hope your doubt is cleared.
Wish him well.
