Suggest Treatment For Pain In The Groin Area During Urination

Question: I have to go to the bathroom 2-3 times nightly. Occasionally, I cannot start a urine stream until I have had a bowel movement. When this happens, it may take 20-30 minutes to start a urine stream. During this waiting time, I'm in extreme pain. Finally, I feel something relax in my groin, and the urine will start and the pain goes away. This has been happening for years. Recently it happened and I inserted a catheter but no urine came out while the catheter was inserted. After about 10 more minutes, the flow started. This happens every few nights.

As an Urologist,i can understand your anxiety.

Your symptoms are typical of an obstructive urinary stream.
This is most commonly seen in prostate gland enlargement,at your age.
The reasons for obstructed urine flow need to be investigated.
Some tests to be done after an examination by an Urologist are :
1. urine routine and culture.
2. blood routine,creatinine,PSA,RBS,LFT,platelets,VDRL,HIVand HbsAg.
3. Uroflowmetry.
4. Ultrasound--KUB,with residual urine and a TRUS.(trans rectal scan).

With the reports of these tests,an Urologist can give an expert opinion.
You may send the reports here,for an expert opinion.

In the meanwhile,you may start Tab.Alfuzocin at bed time.

What are RBS, LFT, and VDRL tests for?

What will creatinine and HbsAg tests show?
I've tried Tamsulosin and Avodart. No help. Is Alfuzocin different from those?

RBS is for detecting blood sugar level.
LFT is liver function test.
VDRL is to rule out a STD.
Creatinine gives an idea about your kidney function.
HbsAg is also part of STD panel test.

Alfuzosin is more effective than the two you've tried.

No chance of STDs. Blood sugar is OK. Lipids are OK.
Would an ultrasound show scar tissue from the TURP?

Or if prostate has grown back into the bladder?

Could botox in my prostate and/
or bladder help my condition?

Residual prostate gland is most probable cause.

1. An ultrasound-KUB with residual urine and a TRUS,will give a lot of information.
It'll point to several causes of your symptoms.

2. One common cause for recurrence of your symptom,is an incomplete TURP.
The other is a narrowing at the bladder neck,causing obstruction.
Scarring leading to stricture at surgery site,is another cause.
That's the reason for you to undergo the tests listed in my reply.

3. Botox isn't an option to relieve your symptoms.

Why would botox not be a viable option? I recently spoke to someone that used to train doctors how to inject botox to relax the bladder and shrink the prostate. He thought it could help my condition.

I had an MRI a few months ago on my prostate to check on the progress of a slow-growing cancer. There was no change in the size of the tumor, so my urologist continues to monitor it. Could the data from that MRI be used to obtain the same information that an ultrasound-KUB and a TRUS would reveal?

So finally you are mentioning about your prostate gland being cancerous.
kindly send your PSA level and bone scan report,to be able to continue with opinion.Once the MRI report is seen,i can give an expert opinion.
Also send the biopsy report.

Botox may be used in cancerous prostate gland,in a person not fit for surgery.
