Question : Hello,
My 15 year old son woke up with pain in his left testicle. The left testicle is about 50% larger than the right one and is hard if you squeeze it gently as compared to his right testicle. He said this is a new pain. He just played in his last Lacrosse game last night. He said he did not get hit in the groin.

To recapitulate:
Male/15 - woke up with pain in left testicle - 50% larger - hard -says this is new pain - Lacrosse game last night - no hit .....
My thoughts:
This may be an emergency situation as this can be due to torsion of the testis and I would advise in such a situation the following:
Report to ER.
Urgent clinical evaluation
Urgent ultrasonography with color mode too to decide the reason.
This is so as if there is a torsion, needs very urgent surgery as it takes only 6 hours for the testis to stop functioning after torsion.
If torsion, needs an urgent reversal surgery.
If there is orchitis or so, needs urgent medical management and investigations of the blood and urine should be done.
