Suggest Treatment For Pain In The Right Upper Quadrant

Question: Dear sirs,

Last week l had a fish meal for lunch and after a while I felt pain reaching from just below the right ribcage to my right chest, after a while the pain went intolerable and when I went home I vomited for 3 or 4 times I also had visceralgine for the abdominal pain.
next day the abdominal pain was better but since then I have been having prolonged episodes of laboured breathing and difficulty breathing that is very annoying accompanied by dull to mild chest pain on both right and left sides, This has been going on for a week

what could be the problem and what should I do?

Hi.
Thanks for posting your query at HCM.

Noticed that your experienced pain in the right upper quadrant of abdomen ( below rib-cage) last week. Vomiting accompanied with pain. Intensity of Pain decreased in the following days, however now you have difficulty in breathing along with pain in left and right side of chest.

Firstly, the differential for your last week symptoms ( after eating fish) is gastritis, OR liver pathology OR gall bladder pathology.

However, if symptoms are relieved , so observation is required and avoid eating that particular species of fish and junk food and unhygienic food.
If the pain and symptom reappear then You may visit treating physician and have following blood test done : 1. Blood Routine 2. Liver function Test

Secondly, for your current symptoms, labored breathing and chest pain, its better to exclude lung pathology. your advised to have following tests done:

1. Chest X ray
2. Blood routine
3. treating physician may prescribe pulmonary function test

I had blood tests about 8 months ago that say that every thing is OK they included liver function tests on the other hand I had an ultrasound back in 2013 that revealed gallbladder stones , can the gallbladder stones be the cause of all these symptoms?

thanks for getting back to me.

if you had blood test including LFT 8 months ago, then NO need to check the liver enzymes now.
however, your advised to have an ultrasound examination of abdomen to rule out any gall bladder stone or stones in common bile duct.

and yes, gall stones do cause pain on the right side just below the ribs . Vomiting maybe present too. However, one needs to have an ultrasound to confirm the diagnosis.

further more, if you have a history of gallstones, your advised to cut fat in your diet and avoid junk food.

its pleasure to be of help and answer your concerns.
ultrasound shows three abnormalities.
1. gallbladder stones
2. colonic gaseous distension
3. mild prostate hyperplasia

lets discuss three separately.
firstly, "asymptomatic gall stones with normal gall bladder morphology usually require NO treatment. observation is sufficient. however , you may take second opinion from general surgeon. Medical management maybe advised and no need to surgical remove gall stones as per your current presentation.

secondly, colonic gaseous distention may require diet modification if your not suffering from diarrhea/constipation. take fiber rich diet, do not eat mouth full and OTC simethicone and probiotic medication maybe take after consulting with your treating physician.

thirdly, mild prostate hyperplasia is not of much concern. your advised to decrease frequency of sexual activity and also consult q urologist gor further management.

last but not least, as mentioned previously, cut fat diet, and loose weight (if over weight) and monitor your lipids.
