Question: What are possible causes of pain under sternum and in middle of back upon waking in the morning?

Acid reflux disease

Detailed Answer:
Hi and thank you for asking!

The most frequent causes of pain under sternum are as follows:
- acid reflux (GERD)
- spine cord postural disorders (mostly back pain)
To my opinion you need to use PPI'S like prilosec OTC plus antacids like gaviscon
IT is too important to keep healthy diet as follows:
- avoid coffee and chocolate
- eat often and small amount of meal
- avoid soft drinks plus coca -cola
- eat 3 hours before going to bed
- use two pillows when you lay down
- increase physical activity
If your symptoms doesn't improve in two weeks than you need to be examined by upper endoscopy
