Suggest Treatment For Pain On Pulling Foreskin

Question: Hi
i had unprotected sex two months ago and my penis was very sore,also i found it hard to pull my foreskin back over my penis head.It was the first time i had sex in five years.It was sore for a few days then it seemed to get better,then i noticed a ring appeared around my penis where it joins the foreskin.There is also a slight discharge under the foreskin and it smells.I bought some cream Canesten Clotrimazole and Hydrocortisone but am frightened to use it in case it causes more problems.Can you advise.

Hi, thanks for posting the concern in the HCM.
As you have stated above, you had difficulty in bringing forward the foreskin over glans after doing an intercourse and thereafter there was a circumferential lesion alongwith oozing and foul smell at the joining of the skin and I have also seen the image attached alongwith. I think a lateral view/ side view would have helped to interpret better and the given image doesn't help to come to a specific diagnosis. Now, you want to know whether clotrimazole and hydrocortisone can be applied or what else needs to be done.

Now, after going through the history and looking at the lesion, I think this may be a case of paraphimosis. Although, you haven't stated a typical history of penile fracture and that is not also suggestive from the image , the same also needs to be excluded by physical examination as you had sore following the intercourse. Furthermore, the area has got inflammed, infected and if urgent care is not undertaken it may lead to sepsis or even local gangrene.

Therefore, urgent evaluation by a consultant urosurgeon is needed.

You will need the following investigations-
Complete blood count, urea, creatinine, serum electrolytes, CRP
Wound swab microscopic examination and culture and sensitivity testing
USG Penis
VDRL
HIV ELISA (after counseling and consent)

Surgical intervention may be needed including aspiration, vertical incision or circumcision.

I don't prefer hydrocortisone cream for this purpose. The response to betamethasone cream along with local application of hyaluronidase have been found to be better than the former. But, you should visit your local urosurgeon first before applying any topical medicine or doing anything further. You have already lost several weeks and delay in starting treatment can put you in danger.

But, with proper treatment the lesion can be cured. However, from the image you have sent, the local tissue visible in the image although inflammed, looks viable. That is definitely a good sign. Urgent surgical attention would help you a lot at this stage.

I really don't think that it can be a balanitis. Perhaps image from a different angle may help, particularly a side view, as I have said earlier. From the available one, it doesn't at all look like simple issues like balanitis.

Secondly, balanitis doesn't present with soreness immediately after intercourse, whereas paraphimosis may occur from a sore after intercourse.

I cannot altogether rule out balanitis from this history and the available image though. It may be there. But, physical examination by a local doctor is needed.

Therefore, you should consult your local urosurgeon at the earliest.
