Question : Every time I eat I experience pain that radiates from underneath my shoulder blade around to my upper stomach, nausea, and diaherrea. This has been occurring for the past two months. My doctor prescribed nexium which isn't working and performed a ultrasound, which showed nothing. Please help. I am getting very frustrated.

Brief Answer:
Possible Pancreatitis- very important diagnoses

Detailed Answer:
Hello,

Thank you for using HCM to post your question.

I read you question and I understand your concern.

What I am understanding from your description, it is that you may have something that it is called pancreatitis, which may be acute or chronic.

It may give this type of pain, and also the diarrhea which it is greasy diarrhea.

To rule out this important diagnoses you will need to follow the below steps:

-- Check Amylase or Lipase, which are pancreas enzymes, and get produced in excess if there is a problem.
-- Check Alkaline Phosphatase which will show if there is any blockage in the biliary ways, which will affect retrograde the pancreas.
-- If any of those comes our suspicious result then the next step which decides finally the diagnoses it is the CT scan of the pancreas and the abdomen.

I hope this is helpful. Let me know if you need more clarifications.

Regards,
Dr. Papaqako

Brief Answer:
Advises of how to treat pancreatitis.

Detailed Answer:
Hello again dear,

Thank you for the follow-up question.

It is a serious diagnosis. Which means that you need to rule it out if you have it or not. Or confirm it and take a treatment. It can be life threatening, and it will need lots of labs and treatments after.
I am not saying that you have it for sure, but that it is important to rule it out as it's the most concerning as per these descriptions.

It is important to not:
-- drink alcohol or smoking.
-- Low fat diet
-- Take Ibuprofen 400 mg or Paracetamol 500mg, every 6-8 hours.
-- Check your sugar in the blood level, because pancreatitis can sometimes cause diabetes.
-- Then of course if the diagnoses its confirmed you will need to take the pancreatic enzymes and steroids as needed.
-- If any of these helps then surgery its the last resource but I sincerely wish it's not.

Let me know if I answered your question.
Regards.
