Question : ive noticed that when i bend over sometimes i will get an excruciating pain in my right upper quadrant and this hard lump forms. its like a very bad cramp . lasts for a couple minutes then subsides. ive had like a ache for 2-3 days because of it.. ive been told i may have a hiatal hernia. how serious is this?

To recapitulate: Female / 62 - excruciating pain in upper right quadrant and '' hard lump'' on bending over sometimes - bad cramp - subsides within a couple of minutes - like a ache for the last 3 days - told as hiatal hernia -Doctor wants to run tests - no medical insurance till March 1 - wants to know how serious is this...

With the history you have provided, this does not looks to he Hiatal Hernia as no such lump can be seen or palpated in this condition.

My probable diagnosis would be :
- Local muscular problems in the said area as your history says about a hard lump and pain like a cramp on bending over sometimes for last 3 days.
- Can think of Costochondritis of the lower ribs. This can be confirmed by pressing fingers on the lower ribs and see for local tenderness. In costochondritis, the local inflammation spreads to the upper abdominal musculature as they originate from the lower ribs.
- Underlying inflammation of the gall bladder / liver / colon or so.

All these conditions can be diagnosed on careful history and careful local examination. Ultrasonography and other tests may be required to confirm or rule out a certain diagnosis.
