Brief Answer:
Can be a muscle tear.

Detailed Answer:
Hello,
Thanks for posting your query.
The appearance of dent below the shoulder can be due to mild detachment of the muscle from the bone due to excessive stress and strain in the area. An MRI scan will help in confirming the diagnosis.
The pain can be stopped with immobilization of the arm. You can use an arm sling for it. Avoid all painful movements. Do take a mild pain killer and ice the area for earlier relief.
I hope this answers your query.

will this require surgery to correct. If so, how long will I be laid up in the hospital and at home?
