Question: My wife, 51, caucasian, can't sleep because of pain in the lymph nodes under her right and left arms. She also has migraines every few weeks and some joint pain in her fingers. She's been to a GP and has had a MRI red but to no avail. There is a history of rheumatoid arthritis in her family (her mother).

To recapitulate: Female/51 - can't sleep - pain in lymph nodes in armpits - migraine - joint pains in fingers - family history of RA - GP - MRI - no avail ...

As per the history you have provided, there are multiple problems which need consideration one by one:
- Lymph nodes: I would advise the following:
A course of an antibiotic, anti-inflammatory medication.
Clinical evaluation, examination and investigations like Ultrasound of the affected area and breasts and FNAC should help to get the most proper diagnosis and help proper management.

Pain in joints of fingers:
X-rays, blood tests including that for RA, an opinion and proper medication by a Rheumatologist will help the best.

Migraine: As you may know this is diagnosis is done by exclusion and classical history and there are 2 types of management. First is a preventive management and the other is symptomatic; an Opinion of a Neurologist, proper investigations and management will help er have proper control of headache.
She should avoid all the factors that instigate or enhance the attack.

This approach of solving the problems one by one helps in rational diagnosis and management.
