Thanks for writing back. The hyperpigmentation occurs due to prolonged inflammation and is part of same process. It should slowly regress with use of Steroid ointment.

                                    Brief Answer:
Kindly get color Doppler scan
Detailed Answer:
Dear Sir
Severe pain in testis ,one also needs to consider the possibility of Torsion Testis (Twisting of testis around it's axis). If Torsion testis is conclusively ruled out using color Doppler scan then there is no cause for concern, the testicular pain should subside with following advise
I would suggest you the following
1. Scrotal Ultrasound and color Doppler to differentiate the cause of pain
2. Scrotal support : Wearing the right size underwear will provide adequate support. If the underwear is too tight it worsens the pain due to congestion and if it's loose it leads to hanging testis and further worsening of pain
3. Analgesic like Ibuprofen will relieve pain
4. Continue antibiotic
Kindly keep me updated about the progress and improvement.
Take care

Deepak
