Brief Answer:
Most likely to be due to Acute Fissure in Anus .

Detailed Answer:
Hello
Thanks for query .

Pain before and after urination associated with pain during defecation is most likely to be due to Acute Fissure in Anus .

Anal and urethral sphincter have same nerve innervation by Nervi irrigantis a branch of Pudendal nerve .So whenever there is pain due to fissure in Anus one gets pain in urethra due to what is called as referred pain.

Consult qualified General surgeon for Rectal Examinaton ti confirm the diagnosis .

The medicines that have been prescribed have little role to play in resolving the issue as there is no infection (UTI) /

Using local anesthetic jelly like Xylocaine Jelly applied three times daily in Anus will help to get relief from pain both in Anus and Urethra.

Take stool softener like Isabgol at night .

Avoid meat and spicy diet to prevent getting constipation .

Ensure to drink ore water .

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

Brief Answer:
Anal pain is due to friction and rubbing of stools against Anal Mucosa

Detailed Answer:
Hello
Thanks for follow up .

The pain in and around Anal region is due to friction of stools against the delicate Anal Mucosa and persistant pain in urethra is due to irritation of the urethral sphincter which last for long time .

I personally do not feel role of pelvic exercise to resolve the issue .

I am sure you will get relief with treatment which I have suggested .

Thanks and Regards.
