Brief Answer:
I think zoloft is either ineffective or notin appropriate dose

Detailed Answer:
Hello,
Thanks for using healthcaremagic.

Although it is not possible to speculate exact reason for feeing bad but the symptoms are clearly indicative of clinical depression. The reason for feeling low could be hormonal changes or the panic disorder itself presenting with depression..

Klonopin generally do not result in secondary depression and I think this is unlikely cause. Having said this I feel klonopin should not be used for long term and you need to discuss same with your doctor.

I also feel that Zoloft is either ineffective or in suboptimal dose range. In my view there should be change of antidepressant if you are already taking maximum possible dose of Zoloft( 200mg).

I hope this helps you.
if you have more questions feel free to write back to me.
Thanks and regards,.

Brief Answer:
There is scope to increase zoloft

Detailed Answer:
Hello,
Thanks for reverting back to me.

Since 100 mg of Zoloft is not sufficient I think you can increase dose of zoloft to 150 mg after discussing with your doctor. In case that too is not effective dose can be further upped to 200 mg after two weeks of 150mg.

I understand that depression and panic disorder are two different entities but high risk of developin another after initial diagnosis of one.

I hope this clarifies further.
Thanks and regards.

Brief Answer:
UTI quite possible and more frequent with hormonal imbalance

Detailed Answer:
Hello,
Thanks for follow up.

The presence of vaginal dryness and lack of sex drive indicate toward lack of estrogen which is part of perimenopausal events. Since the hormones affect the Ph of urogentital tract the infection rates dramatically increases with hormonal imbalance.

I hope this assists you.

Brief Answer:
Yes that too possible

Detailed Answer:
Hello,
Thanks again for follow up.

Perimenopausal hormonal changes are one of the most common contributor to development/relapse of depression during these years. If we go for statistics women during Perimenopausal period have almost 1.5 times increased risk of depression in compare to other age group women.

Although the depression during this period is not different but sometime presents with increased severity and that could be the reason for feeling different.

I hope this assists you further.
Thanks and regards.

There is always hope

Detailed Answer:
Hello,
Thanks for follow up.

I understand your dilemma and feeling of being lost but I must tell you that there is always hope no matter how the condition is.

Please talk to your doctor for increasing dose of Zoloft as there is good possibility of efficacy for Zoloft in perimenopausal depression.

The another best option is Pristiq which is available in dose strength of 50 and 100 mg.

I hope this assists you further.
Thanks and regards.
