Suggest Treatment For Panic Disorder With Anxiety

Question: from past one year i am having extreme fear with extreme negative thoughts like going crazy and now am taking trypomer 25 mg in night and 10 mg in morning and daily at a particullar time i get extreme fear at that time i take epitril orral.

im not able to be happy and this s taking me more as like hell. im having a feeling as i m gng crazy... please help how should i make my life normal.

also please help me to overcome my negative thoughts and i get fear in wat ever i do. if eat outside food my mind thinks it iwll a poison and my symptoms will be heart racing my bp willbe 170/110 and blarred vision no concentration hands become chill please help me. my symptoms are lighthead/dizziness/ floating head and body/not feeling my hand leg...

Thanks for writing to us.

I have read your query and I guess you seems to be suffering from anxiety disorder. It may be generalized anxiety disorder or panic disorder.There are episodes of panic attack and in between attacks anticipatory anxiety is there.

By seeing medicines I can say they are prescribed by psychiatrist. If you have not consulted psychiatrist yet, please do the same soon. As detail mental status examination and history required for proper diagnosis and treatment by a psychiatrist.

If I were your treating doctor I would like to add a medicine form SSRI group which includes Fluoxetine,sertraline,escitalparm,paroxetine. I prefer paroxetine 12.5 controlled release tablets in such cases. I also would like to add epitril 0.25 in morning and noon for 10-15 days to control your symptoms fast.Gradually I would like to stop amitriptyline after starting these.

As you have taken lithosun in past it suggests either bipolar depression or resistant depression. Please follow up with photos of past reports,prescriptions and some symptom history when you were given lithosun, so I can give more helpful advice.

Thankyou sir for your answer.

I was taking lithosun for past 5 months for having the feeling of depression , my symptoms were loneliness, sucidal thoughts , going crazy thoughts ,extreme fear and by this thoughts my physical symptoms started as dizziness, bp increased, less concentration.

My episodes will be every twice a week like head and body floating , no concentration.extreme fear of falling down and going crazy is making me more anxious.

Plese help me sir wat should i do ??? Am i truely going crazy ?

Before identifying the illnesss i have taken all test for mri scan,synis scan,ear scan all were normal. then at last i went to psycatirst.

I am sorry for delayed reply.

You seem to be suffering from severe anxiety with depression.

Please note that you can be normal with help of medicines and psychotherapy.

Please consult psychiatrist personally as soon as possible.

As you have undergone all necessary investigations and all are normal, it is a good sign. You have no specific physical problem.

If I were your treating doctor, I would like to start with clonazepam(epitril) 0.5 mg thrice a day for fast relief of your anxiety symptoms and decrease the dose as symptoms improve.
As I mentioned before I would also start paroxetine Cr 12.5 mg once a day and if necessary would like to increase twice a day.

You might be given lithium for suicidal thoughts. If currently you have suicidal thoughts, I prefer indoor treatment and if needed ECT – Electro Convulsive Therapy can be given.Low dose antipsychotic like olanzapine for short duration is also useful in such scenario.

Simultaneously I would like to evaluate for the stressors/situation which had initiated the problem. After that psychotherapy can be start. Specifically I would like to go for CBT- Cognitive Behavior Therapy.Psychotherapy can be started after your anxiety symptoms are under control.So in your case first line of treatment is medicines.

You can be cured for sure as you have suffered since one year only, so just try to keep yourself calm, and do not take any step which can harm you. Without hesitation go to psychiatrist and start the treatment.

Thank you so much sir for making me feeling better about my situation. If i take epitril i feel at ease. How much i can take per day epitril 0.5.

am also a working women. im able to do my work only with medicines and take so much courage to go to my work. i have many episodes were i had symptoms of dizziness and bp.

I dont have social stress.

Please advice me if i continue my work it will worse my illness or have to continue it for making my mind to deviate from anxiety.

Thanks for follow up.

If you feel somewhat better with my words, I think you can be helped with psychotherapy in very positive manner. It may reduce requirement of medicines.

Epitril is a benzodiazepine- anti anxiety medicine. In higher dose it is useful for sleep disturbance.
In your case initially I think you can take 0.5 mg three times a day. After 3-4 days of stability you can reduce it to 0.25 mg in morning and noon and 0.5 mg at night. If you feel sedation after starting, you can immediately reduce to 0.25 dose immediately.And even if with 0.25 if you feel sedation, you can take half tablet of 025 mg.

One alternative is etizolam, an anti-anxiety-benzodizepine, with least sedation. If it is available there, ask your doctor about it. It can be taken as same doses like epitril.
These medicines are for symptomatic treatment, overall cure can be achieved with SSRI medicines and psychotherapy.


If you can concentrate at work and do it properly without any problem, you should continue your work. It is not going to worse your problem rather it may help by diverting your thinking process in fruitful direction.

If you do not have any stress, some physiological cause of depression/anxiety should be ruled out. Specifically I would suggest, you should go for thyroid testing if note done yet.

Thank you sir for so much in details . I really appreciate that you have given some much information abt my illness and taken so much time to explain it all.

I have done thyriod test also . i dont have thyroid problem.

My questions are -- Can anxiety lead to crazy ?can i continue taking tryptomer?

1)My symptoms started before a year where i watched a horror movie and i coudnot sleep for 30 days due to imagining of devil appearing but actually not . I used to sleep for an 2 hour per day for 30 days .where the symptoms started as dizziness following fear , anxiety , many negative thoughts was the incidient where the anxiety happened .

2)Another incidient for depression was loss of my dream job and relationship breakup.

3)as u perfer i will take epitil thrice a day and i will visit doctor for theraphy and to take SSRI medicine.

4)as u said i will continue my job.

As your problem is started after seeing a horror movie, it suggests Post Traumatic Stress Disorder(PTSD) – a type of anxiety disorder.

Yes anxiety can make person crazy for a while.

You can continue tryptomer if you feel good at sleep with it. Otherwise it is not so much helpful.

Loss of job and break up issues are common stress factors which lead to depression. Psychotherapy – CBT have superb results in depression treatment in such cases if patient is well motivated.

Yes epitril will help you to calm down. SSRI will treat your negative thoughts and low feelings. These medications are prescription medications, so you need to discuss this face to face with a psychiatrist.

If you can continue your job without any problem, it suggests your depression is not severe and you can be recovered fast.

Hope you will take a help of a Psychiatrist soon and cured.
