Suggest Treatment For Paranoid Delusion

Question: I was depressed 3 years back. I had a misconception that my colleagues were playing some politics against me. When I approached doctor he gave me some injection and prescribed me some medicine. I don't remember all the prescription but 6 months back he gave me sizodon forte and now I am taking Oleanz rt 15.

Now I don't have any problem. Due to my suspicious behavior I was taken to doctor. Now I want to stop the medicine after 3 years.

Can I stop medicine. I fell that I don't have any problem now. My family members are forcing me to visit psychiatrist and continuing medicine. Please advise me. I strongly feel that I don't have any problem.

Hello,
The symptoms you mentioned are suggestive of psyhosis (paranoid delusion). This is treated with antipsychotics , usually oral tables suffice. But you have been given injectables initially and finally you have settled on 15mg olanzapine --both of these reflect the moderate to severe nature of disorder.
In such patients it is good to continue the treatment for few years (after the patient has achieved complete response).
You mentioned that you are fine now (means you are going well with others and not disturbed by thoughts of harm from others etc.). You can talk to your doctor about reducing the meds and not stop it on your own. Stopping the drugs suddenly (even after a period of few years)can bring relapse ). So, its better to meet the doctor and ask him to reduce the dosage. Take that dosage for next few months and if you continue to do well --your drug dosage can be reduced further. Like this, you can achieve either drug free status or reach a minimum dosage level which keeps you good.
The only problem with psychiatric illness and other bodily illness is that in psychiiatric illness many times the power of judgement is clouded and patient cannot see or feel the truth. so, always trust your close friends or family and doctor as they will do their best to keep you good.

Dont worry, if you are asymptomatic for more than an year now, meet doctor and request him to try reducing the drug.

How long to continue more? I am looking for my marriage alliances now. I will not get good response if I tell that I am on medication. How to tackle this situation. Should I hide this disease or I tell openly. If I am open according to XXXXXXX mentality I wont get positive response from girl side. Please Advise.

Hello,
As I have mentioned, you should ideally go for dosage reductionand take that for 2-4 months. If you continue to keep fine and asymptomatic then further reduction can take place. So, like this, if you continue to keep fine your dosage will be all done in 1- 11/2 years. One may do it fast by reducing dosage every two months but it is done under strict supervision ( because it carries risk of relapse and then the drug will be continued for whole life even if you remain asymptomatic).

Second question regarding matrimony is very important. You are very right that when you tell the girl side about your illness -many good alliance will reject you (may be 8 out if 10), but the one or two allaince which are left (who will accept you) will be the one who will definitely and willingly stand by you all your life - both in disease and in good health. Those one or two alliance will stand by you all your life, even when your parents/siblings might not/cannot be there. Its wise to enter into a life long relation by being truthful.
You are doing well now, so, do tell about your illness.
I have seen several cases of psychiatric patients and its an overall judgement that matrimony done with truthfullness is mostly helpful ahead in life. But when done secretively and hiding the illness, the results are grave and may even lead to divorce ( which further acts as a stressor and may lead to relapse) once the secret is out.

So, take your own decision after giving a thought to what I mentioned.
