Question: Dear Doctor,

My son is having Large Perimembranous VSD With bidirectional Stunt disease in his hearts as per the doctor in Dubai.

Kindly let me know anything is serious,

I have read through your query in detail.
Your little one has a congenital (by birth) heart disease termed perimembranous ventricular septal defect, commonly called as hole in the heart. This is a serious condition but with advances in medicine, it can be corrected by surgery and is best done after 6 months and before he completes one year in age.

There is a persistent opening in the wall between the right and left portions of the lower chambers (ventricles). This is causing mixing of blood and your baby needs special attention and regular checks with the cardiologist.

This condition is treated by surgery best after 6 months of age. After 6 months of age, the success rate of surgery is very good. It is said that infants and children with large VSDs have a good prognosis. Optimal medical management, with appropriate timing of surgical intervention, has the best outcome.

Please be in regular follow up with your doctor and discuss about surgery and when they would like to do it, usually done between 6 months to one year.

In case of small VSDs, there can be spontaneous decrease in size and closure by around 6 months of age in about 50 percent patients. Since your son is having large VSD, the possibility of self closure is less. Anyways a medical follow up is done in every XXXXXXX till at least 6 months after which only decision of surgery may be taken.

Hi Doctor,

one more thing my baby weight is not gaining what can i do for this,any medicine, and in the report doctor mention PA Smell to be small & he have TOF disease also,what is TOF.Same doctor said large vsd will close when he growth its smaller.

TOF is a combination of 4 heart defects.
Pulmonary valve stenosis
Ventricular septal defect
Overriding aorta
Right ventricular hypertrophy

In this condition the blood does not contain adequate amount of oxygen and the baby will have problems due to this. Babies who have tetralogy of Fallot may tire easily while feeding. Thus, they may not gain weight or grow as quickly as children who have healthy hearts. Also, normal growth depends on a normal workload for the heart and normal flow of oxygen-rich blood to all parts of the body.

No medicine can cure this and surgery needs consideration. A small PA refers to small pulmonary artery, a vessel that takes blood from heart to lungs.

You may consult doctors at a speciality heart hospital and get expert advice. Treatments need to be discussed after doing clinical examination for your baby. There are well known hospitals in XXXXXXX and abroad where proper guidance by experts is available.

TOF does not recover by itself and surgery is being increasingly done at infancy to correct this condition. The success rates for this surgery is 95 percent.

Studies have shown that the survival for untreated Tetralogy of Fallot is approximately 75% after the first year of life, 60% by four years, 30% by ten years, and 5% by forty years.

For the above reason it is best to take more than one opinion and decide on corrective surgery if recommended, after clinical examination.
