Suggest Treatment For Persistent Back Pain And Frequent Urination

Question: I have a family history of kidney troubles. My mother died of renal failure, her mother died of chronic pyelonephritis and her grandfather died of hemorrhagic nephritis. My father had recurrent kidney stones. Three of my five siblings have kidney issues of one variety or another. I have had kidney stones and have recurrent UTIs. (I am male, age 52). I have had, and continue to have bloody urine. This occurs occasionally, usually after some physical trauma or exertion such as riding in the car for several hours (but not always) sitting on the ground for a long period of time, running, digging or raking in the garden, etc. Usually the urine is dark brown for one voiding and then normal. Most recently, it was bright red (that has happened before) and lasted for three voidings (18 hours). I have persistent back pain centered at the location of the lower edge of my right kidney. Pain radiates from there up to the top of the kidney or more often down into my flank and around into my groin. I sometimes have severe SI joint pain. It is so severe that I will be unable to stand after sitting for 30 minutes or so. I have frequent urination (daytime only) and lower volume than normal and I have difficulty finishing voiding- I have to massage my penis to get the last or it dribbles.

Since you have had strong family history and have complaints as mentioned above, you must have been to many Doctors and must have investigated.
Please post these details and the reports if you may. This will help us for further decision.

It is difficult to say whether the urinary stones caused UTIs or recurrent UTIs caused the stones. Also needed is to have all the relevant investigations.
Please give the details so that we can discuss the problems in details.

Recurrent stone formation needs additional investigations related to parathyroid glands and hormone.

My father had recurrent kidney stones. I have had kidney stones only once- 12 years ago. I do have occasional kidney stone-like pain sometimes, but my current complaint is the kidney pain. It is an achy pain centered at the lower edge of my right kidney.

When I lay on either side, I have an internal sensation of weight near my kidney putting pressure toward whichever side I am lying on.

In the past month, my morning urine volume has decreased to less than 100 ml. I drink between 1/2 and 1 gallon of water per day.

I have not been to a doctor. My PCP died and I have not been able to see a doctor since as none are accepting new patients with my insurance. A specialist would, but they require a referral.

You had kidney stones 12 years ago, have occasional kidney stone-like pain, now have kidney pain, achy one centered around lower edge of right kidney and as explained further by you.
I can understand the problems of insurance and so on as you have mentioned.
But ultimately it is the investigations that will help to clarify the situation, without which no one can give you an appropriate treatment.
With all the symptoms you have mentioned, there is a strong possibility of urinary problems that need investigations, clinical evaluation and proper treatment in view of a strong family history related to the kidney problems.
I would advise you the following:
Tests of blood, urine and stool.
Ultrasonography of the abdomen.
And clinical evaluation to correlate the symptoms, signs and the reports to treat you properly and go for further investigations if required.
