Question : xxxxxxxxx have had chronic depression for 8 years and with treatment of various kinds, have improved considerably. But lately have been having a strong feeling of "out of body" feeling strange toward my wife, like "Who is She" and toward myself, "Where is my personality" I fake very well and don't feel so bad from depression, but these feelings seem to be getting worse, if anything."

What you are describing is probably depersonalization and derealization. Given your history (which I have carefully reviewed), this could be from a few different things.

First, it could be simple anxiety. Anxiety can cause these symptoms, and is often comorbid with depression. Vyvanse can worsen anxiety, so you might want to revisit the need for this.

Second, it could be a symptom of depression. For this particular symptom of depression, a medication like Risperdal would work well to help you feel more normal.

Third, it could be neurologic, such as frontotemporal dementia. I would visit a neurologist and undergo cognitive testing to rule this out.
