Suggest Treatment For Persistent Fever In A 4 Year Old

Question: My daughter 4 years old caught fever yesterday and we gave calpol 250mg with 4 hours gap and so far have given 4 times. However fever seems not reducing and temp is at 102 and after sponge body with lukewarm water temp is at 101.
Do you suggest to go emergency for this case or can wait till morning opd?

Sorry for the minimal delay in answering as the amount of doctors available deep in the night (in India). However, I would try to answer without delay. If temperature is not controlled with calpol (paracetamol), giving Syrup Meftal P (mefenamic acid) may be more effective in reducing the temperature. To comment on the actual dose required, I would need to know the child's body weight. You have done the right thing by giving sponging with lukewarm water but let the water dry off from the skin on its own. Most of such fevers are due to viral infection but if not controlled you would need to visit a doctor without delay. High temperature if not controlled soon, may even lead to brain damage and convulsions. But let me assure you that if you can procure the medicine I mentioned, the temperature would probably come down.

Thanks fortunately we had meftal p and have given 5 ml and seems like temp is getting control. Her weight is around 15kg. We gave the medicine an hour back when do you suggest to give next dose and any other medicines you suggest for her. She also has cough so I'm thinking of meeting a pediatric nearby, please suggest, thanks for your help

Thanks for asking back. The recommended dose for your child would be 3.5 ml but it is not a problem if 5 ml is given. Ideally it is given at 8 hourly intervals but can be given at a minimum interval of 3-4 hours. Give it only if the child has fever and not on routine basis.

Give the child steam inhalation. Just sit with the child on your lap and with a source of steam before you. The water vapour will be in the air and automatically the child will breathe it in. It will help in diluting the cough. Giving Syrup Relent 2.5 ml twice daily would be helpful. Such viral infections generally take 5-7 days to resolve on its own and irrespective of the use of antibiotics.
