Brief Answer:
You should consult a Surgical Gastroenterologist.

Detailed Answer:
Hi
Haemorrhoids might be in the advanced stage (Grade 4)where medicines won't work.
You should consult a Surgical Gastroenterologist with MCh Degree, not a Medical Gastroenterologist.
Stapler haemorrhoidectomy is the latest and best modality of treatment.
Till then, take high fiber diet, drink plenty of liquids.
Eat fresh fruits and vegetables.
Your stool should be soft.I prescribe laxative containing Lactulose 10ml at bedtime for stool softening to my patients with similar complaints.
Hope I answered your query. If you have any questions, I will clarify further.
Wish you good health.
Regards
