Suggest Treatment For Persistent High Fever In Child

Question: My daughter is 3.5 year old 13 kg weight. Her fever reaching 102 in each 4 hr. Last time in may 2015 she had feve reaching 105. Also has vomiting tendency during eating. Even 12 ml of calpol 120 mg is not proving efficient. Now child specialist XXXXXXX said give 250 mgcalpol whenever fever comes. Why calpol is not proving effective. What 2 do

Although the exact mechanism of action of paracetamol is undefined, the primary mechanism of action is believed to be inhibition of cyclooxygenase (COX), with a predominant effect on COX-2. Inhibition of COX enzymes prevents the metabolism of arachidonic acid to prostaglandin H2, an unstable intermediate byproduct which is converted to pro-inflammatory compounds and pyrogens which are often responsible for the rise in temperature during fever. In the central nervous system, inhibition of COX enzymes reduces concentrations of prostaglandin E2, which lowers the hypothalamic set-point to reduce fever. Sometimes when this action of paracetamol is not adequate as compared to the action of the pyrogens, fever no longer gets controlled by paracetamol. In such a case, the use of mefenamic acid (Meftal P) in doses 4-7 mg/kg/dose at minimum interval of 6-8 hours can provide relief from fever since it is more effective compared to paracetamol.

The fever needs to be brought under control anyhow. Tepid sponging can be helpful.

4ml of Calpol 250 is the right dose for your child. The dose of Calpol and Meftal P are unrelated and can be given independent of one another. So an interval of 3 hours is absolutely fine.

This is separate trouble. My daughter's fever has gone away.
My daughter (3.5 year 13 kg weight) for last 2 to 3 months saying that she has sometimes pain in inner right/left side teeths. one day at one side, other day other side. but with open eye no swelling/cavity could be seen The pain resides in own natural way. I did not give importance. But from 2 days ago I saw below front right side spade type teeth reddish swelling upto root of her teeth with pain. I gave BETADINE ORAL soution for mouth wash and Metrogyl gel at the point. after that pain I think gone away. But reddishness and swelling remaining. Any ANTIBIOTIC TO BE CHARGED OR NOT. wHAT SHOULD BE the DOSE? If any common medicine which is available in all area prescribe me. What has happened to her? No swelling at mouth yet?

Thanks for asking back. I have gone carefully through the details. Our mouth contains bacteria which degrade food leftovers and thereby may cause gum soreness. Probably that is the cause of the problems you mentioned. You have done the right thing by topical application of Metrogyl gel and the use of Betadine mouth wash. But it is more important to brush twice a day and to maintain oral hygeine. The brushing should be done by brush strokes starting from the junction of the teeth and gum and gentle upward strokes should be used for the lower teeth whereas downward for the upper set of teeth. A google search will let you know details on the correct brushing techniques.

I do not think that antibiotics are essential. So the question of prescription or dosage does not apply. Use of Chlorhexidine or Listerine mouth wash should be helpful.
