I am Dr Nishad BN resident orthopedician answering your query. Tietze syndrome is effectively treated by orthopedician.
You can apply NSAID gels like ketoprofen or diclofenac and methyl salicylate, capsaicin combination on the affected area. If this is not helping then you can take NSAIDS like Diclofenac or ketoprofen orally. Giving warm compress or ice pack is also good. If this is not helping then local injection of corticosteroids or anesthetic will be helpful.
Regarding pain on lower back: if you provide some more information it will be more helpful.
When it started. ?
Do you lifted heavy weights or did unusual exertion. ?
Is there any radiation of pain towards lower legs or knees. ?

Follow up: Dr. Nishad BN (13 hours later)
when breathing,coughing,sneezinng heavy pain comes in chest and the back quite
opposite of chest(back side of upper rib). i already used the medicines like overon, diclofinac , nimslide tablets orally and( applied gels) externelly. but no use from these medicines.
i suffering from this problem for 6 years.
so please suggest best treatment

Local injection of corticosteroids or anesthetic will be helpful in such case. You can approach an orthopedician for this. Along with this you should also go for physiotherapy. Physiotherapy like short wave diathermy and ultrasound will be beneficial for this. You can also do infrared therapy and warm compress.
