Suggest Treatment For Persistent Post Nasal Drip

Question: I am suffering from Post Nasal drip for the last about 15-20 year.Some times my problem aggravates.My problem has been aggravated for the last about two months back and Post nasal drip is taking place on the right side on my head and thick mucus is dripping down to my right ear and throat. There is heaviness in my head due to this.
I had visited an ENT specialist,who got my CT Scan done which showed that a soft tissue density polyp measuring 16 mmx16mm in the inferior wall of right maxilliary sinus and also deviated nasal septum with bone spur. Besides this the inferior turbinates are also hypertrophied.(Images and reports uploaded)
He had also got my IgE level test done which was 443 IU/ML.
He diagnosed Allergy sinusitis and prescribed me Taxim OZ anti biotic and Mondeslor anti allergy medicine .Besides this he also prescribed Ezicas-F nasal spray and Rhinoset decongestant nasal spray and steam inhalation.
.However,I have not got any relief .During my second visit he told me that there was no need for any surgery of my sinus polyp and deviated nasal septum and said that Post nasal drip is taking place due to allergy. But I failed to get relief.
During my second visit the doctor told me that Post nasal drip is taking place due to Allergy.
Thereafter I visited another doctor who prescribed short course of oral steroid predinisol- 20 course for one week along with steroid nasal spray Efnase and anti allergy tablets..I had completed that course also .But I failed to get any relief.I have tried all the anti allergy medicines but failed to get any relief.

Kindly suggest me effective treatment for my problem.Also let me know whether allergy is the root cause of my Post nasal drip.
Kindly also let me know wether poly in inferior wall of my maxilliary sinus can cause abnormal secretions.
Do you think that now surgery is an option in my case for the removal of poly,correcting deviated septum and bone spur and hypertrophied turbinates.
I want to made it clear that I am not feeling any notable nasal stuffiness due to post nasal drip but heaviness in the head is bothering me quite a much.

Hello and welcome to healthcaremagic,

I have gone through your history and seen your CT scan. You are suffering from allergy in the nose. The treatment you have recieved is correct.

The mainstay of allergy treatment is local steroid nasal spray and antihistamine and antileucotrine tablets.The most important thing to remember in allergy treatment is that it takes time to get relief, typically it can take upto 5 weeks for treatment to show its effect. So, do not worry, continue with the steroid nasal spray.

Surgery can be considered only if there is nasal block. There is no need for surgery if your nose is not blocked. Surgery serves no other purpose in allergy. You can also use normal saline nose drops. 10 drops 5 times a day. It helps to keep your secretions thin. do not worry about the polyp in your sinus. It is not significantly large to cause symptoms.

I am feeling secretions dripping down only from right side of the head in to ear and throat .Why it is so? Is it due to deviated nasal septum and bone spur on right side?

I have been on anti-allergy medicines for the last more than two months but failed to get any relief .Even oral steroid failed to provide me relief.
Kindly suggest me the name of the company of saline Nasal drops.

Hello and very sorry for the delay,

Deviated nasal septum is not responsible for secretions only on one side. Amount of secretions can vary in amount and consistency on both sides. Also there are routes by which secretions travel from the back of the nose to the throat. They may not be similar on both sides.

When the diagnosis of allergy is in doubt then you can get skin and blood tests done. Skin tests are more sensitive than blood tests. All the common allergens (allergy causing agents) are tested in this way. I suggest you get these tests done. These tests are not routinely done in all allergy patients, they are done when when we are unsure of the diagnosis. These tests are also done when immunotherapy is considered for treatment. Please talk to your doctor about it.

You can use normal saline bottles that are used routinely for fluid replacement in any hospital. Just ask for normal saline bottles in any medical store.

Some ENT doctor had told me that the polyp measuring 16 mmx16mm in the inferior wall of my right maxilliary sinus could be the cause of abnormal discharge although it is not causing any obstruction in the sinus.
I do feel that this might be the case .What is your opinion?

Yes, it could be due to the polyp in the right maxillary sinus that is causing the abnormal discharge.

You could go for a functional endoscopic sinus surgery (FESS), if required.

While doing surgery if the surgeon feels that the septum is considerably deviated or if the spur is found blocking the airway then it will be taken care of during surgery. Do not worry.

If the cause for the increased mucus production is indeed the polyp then suregry will definitely help you.
