Brief Answer:
Topical potent steroid and oral cetrizine

Detailed Answer:
Hello. Thank you for writing to us

I am dr.kakkar (dermatologist).
I have gone through your query and i have viewed the image.
This seems like an eczematous dermatitis. Nystatin is an antifungal but thus does not seem like a fungal infection.
If i was the treating doctor I would suggest you to use a potent topical steroid cream e.g clobetasol propionate 0.05% cream, twice daily.
In addition you may also take a tablet of cetrizine 10mg once daily for symptomatic relief from itching.
