Suggest Treatment For Persistent Vomiting, Weight Loss, Low Appetite And Fever In Children

Question: Good Noon Sir,
My child XXXX is 7 months old and has present weight of 6.3kg.
He is vomitting from thursday and having low grade fever.
I have visited pediatrician on saturday and pediatrician says it is viral infection and advice SWICH syrup for 6days 1/2spoon.
On saturday XXXXX got diarhhea and fever, white stools two times.
On sunday no fever, no diarhhea, I gave him Zinconia and Enterogermina alongwith SWICH.
But vomiting still persists. He did atleast two forceful vomit in a day.
XXXXX has losen 120gms weight in last 4 days.
XXXXis not eating anything except breastfeed and water, not even ORS or juice.
When I touch his upper stomach, I can feel running sound and if i press his upper stomach he vomits or feel uncomfortable.
Today i visited pediatrician again and he says that XXXXX has tonsils too and pediatrician recommends to give Moxikind CV for six days and alogwith Tixylix New for 4 days.

What will be main cause, as he is not eating anything and I am a lot concerned.

Welcome to the HCM

I have gone through your question and understand your concerns. It seems that your child has been suffering from a viral or less likely a bacterial gastroenteritis episode. Antibiotics and zinc syrup along with the infection can lead to gastritis in infants which presents as vomiting episodes as in your baby.

I would recommend you to start him on an antacid such as Digene Syrup or Junior Lanzol dispersable tablet and antiemetic such as ondansetrone or Domperidone.

You may continue the treatment for tonsillitis along with it.

I would like to know a few things to guide you further:
1. The temperature spikes - if they persistent and more than 100 degrees Fahrenheit. And what is the frequency.
2. Is there persistent white stools.
3. How is the status of tonsillitis.

You may send me any reports if you have with the above answers so that I may help you further

Dear Mam,

Thanks for your valued reply.

1. No fever from sunday.
2. No white stools now a day, white stools are observed only on saturday.
3. Pediatrician identified tonsilitis today only, I can not see it.

I have emeset syrup and i will purchase Digene today.
What are the doses to give?

Should I discontinue SWICH and Moxikind CV?

I am at office, so could not provide you doctor's prescription.

At present XXXXX is not eating anything except breastfeed.

What should i feed him?

You may hold antibiotics if mild tonsillitis without fever

Hi
I recommend Digene at 2.5 ml twice daily dose and Syp. Emeset 2.5 ml three times daily to my patients of this age group.
Ensure offering regular feeds and oral rehydration therapy such as lemonade, ORS, soups, buttermilk etc. to keep the child well hydrated.
Most cases of mild tonsillitis without fever do not require antibiotics. So, I would recommend you to hold antibiotics at present. Also, zinconia can be held till vomiting subsides. You may continue Enterogermina.

I have discontinued Moxikind CV and SWICH.

I am giving Emeset, Tixylix and Digene to XXXXX.

However today he got diarhhea, loose poop, yellow color, nine times since morning.

Also he not playing well.

You may give Racecadotril granules for frequent stools

Detailed Answer:
Hi
As the frequency of stools is still persisting without fever, I would recommend you to start him on racecadotril granules. It reduces the hypersecretion of water and electrolytes into the intestine and thus helps in controlling loose poops.
You may give 10 mg racecadotril granules thrice daily till 2 formed stools ( maximum up to 7 days).
Continue Enterogermina for 5 more days and if the vomiting has improved, restart Syp zinconia 5 ml to complete14 days.
Keep him well hydrated by feeds and oral rehydration therapy especially ORS as advised earlier.

Hopefully this will help you. I'd still you feel the condition is worsening , then consult a pediatrician for proper clinical examination..
