Brief Answer:
Peripheral or central vertigo, needs to be determined

Detailed Answer:
Hello and Welcome

I appreciate your concern

Based on the symptoms you describe, with such a vertigo episode accompanied by chest pain that is involving the right, ER assessment is required. Cross syndromes need to be looked for on the neurological assessment. By Cross syndrome it means that e.g there will be a cranial nerve palsy on right side and there would be ataxia on the left side. If such deficit is determined then CNS cause related to the vertigo should be looked for by ordering a CT brain or MRI , to rule out a stroke, primarily. If there is hearing loss, tinnitus with no focal deficits and nystagmus coinciding with peripheral causes then causes such as BPPV, menieres disease or labyrinthitis should be considered and treated accordingly.

Wishing you best of health

Thanks
