Suggest Treatment For Phimosis

Question : Hi..doctor its embarassing to tell my problem but please help me.. actually recently i came to know that i suffer from a condition called phimosis ..i am 22. It was shocking. I then searched all over the net and did some stretching and now i can pull my foreskin but it comes out a little but not fully and if i force it i fear it may cause a condition called paraphimosis which is more dangerous. what to do i cant tell anyone or go to doctor. will stretching work, i feel its a little less tight then what it was before..please suggest alternatives to surgery like something i can do at home and get it cured by using cocunut oil?.is this a huge problem? i am very worried

Dear Mr Subbu

There is nothing to be embarrassed or worried about. It's a common problem faced by young individual.
Do not worry it can be taken care off non surgically.
Do not forcefully pull it back for two reasons
1. It will lead to paraphimosis
2. Raw surface created on both prepuce and glans and cause further adhesion

I would suggest you the following
1. Hot Seitz bath : To a tub of lukewarm water add handful of common table salt. Sit in it for 15 to 20 minutes. Gently pull back the skin and clean it. Remove the white secretion which is known as a smegma. Pull it back slightly and not completely. Over a period of time you will be able to retract in completely. Replace back the skin and take a bath.
Do this procedure twice a day.
2. Local application: After bath, dry the body. Take soframycin and Betnovate ointment. Empty both the ointment tubes In a small clean plastic container and mix well.
Gently pull back the foreskin and apply the ointment mixture and replace back the skin.
Apply it twice a day for 3 weeks.
Betnovate is steroid ointment and will prevent future adhesion and will help in gentle retraction.

Majority of my patients benefit from it . If phimosis still persists , simple out patient procedure like prepucial separation will help.

Sir, I have just started job recently and stay with 2 other friends in a house and its difficult to arrange hot seitz bath but will try to do it .and sir where will I get the betnovate cream and without prescription from doctor is it possible to get it? and i feel this condition is affecting me mentally and i cant share it with anyone..If for example sir i forget about this condition for some time..will this problem of retraction be automatically solved or is stretching everyday very necessary my age is 22.sir is there any side effects of applying the bethnovate cream? and i am worried about the photographs thing...thanx in advance

Hi

Betnovate is a steroid ointment and available over the counter easily. I hope you are residing in XXXXXXX
If the phimosis is not worrisome, you can forget it for time being.
Alternative to seitz bath is while taking hot shower or hot water Bath , retract the prepuce and clean it. After bath apply Betnovate plus Soframycin as advised. Local application of Betnovate is safe and will not cause any systemic side effects.

Sending photograph is totally safe and professional.

Thanx sir for replying me. I live in XXXXXXX sir i have tusted u and sent photograph so that you can assess my condition better please help me regarding this. this is the maximum i can retract after which i can retract but i am worried the foreskin wont come back .is my condition normal and not too severe .actually i have no idea regarding this.if i use all you have suggested how much time is needed to retract fully i am a little worried .

Your condition is okay. There is no cause fro concern. The urinary opening is easily seen so there is no cause for concern. Continue my previous advise you should be able to retract completely.

Intitiation of sexual activity will also normalise it. There is absolutely no cause for concern.
