Brief Answer:
Moderately severe Paraphimosis.

Detailed Answer:
Hi Sir.

I understand your query and concern.

You seem to have long standing Paraphimosis -irreversible foreskin retraction.

This seems to of moderate severity in your case.

You need to undergo circumcision -removal of the constricting foreskin coupled with/without strictureplasty if any.

This condition might have resulted either from forceful retraction of the foreskin or from injury during vigorous sex.

All you currently need is an urosurgical consultation at the earliest.

Baseline renal function tests will be handy before consultation.

Thank you.
