Suggest Treatment For Phimosis

Question : Dear Doc,
I am 31 year old, I have been suffering from phimosis since childhood. Its very tight at the tip of it. Since i want to get rid of it, but i have read that circumsission is very painful and it leads to loss of some sensitive feeling. Rather i would like to know whether there is any cream by which i can loosen my foreskin and stretch it.
I am also very worried as i am not able to expose my glands and hence not able to clean it. So definately its not hygenic.
3 days back i got little pimple like things on base of my penis. Maybe i felt it is folliculitis and i have applied surfaz sn cream and now it seems better with it.
Also let me know what possible ways in which i can clean my inside foreskin and prevent it from damage.
Doctor please note that i am G6PD so i am always afraid to take unknown medicines and creams.

Hi

I understand your concern regarding your skin problem and I hope I am able to help you in the same.
Your basic problem is that of phimosis leading to difficulty in skin retraction.
After going through your complete query ,my viewpoint is that you should consider getting Circumscision done by an experienced surgeon.
I would like to tell you certain facts .
Circumscision ,when done by an experienced surgeon ,does not cause any sensation loss ,infact it improves ones sexual well being due to exposure of the glans. This procedure can be done under anaesthesia in a day care centre and post operation healing occurs in 7 to ten days .
Now the reason why I am suggesting you this procedure is that since you have this problem since childhood,no creams would help in your case .These creams are beneficial only in young children in the early cases.
Also since retraction of the prepuce is not possible , it is almost impossible to clean the glans completely .In such cases ,there are high chances of recurrent infection ,also it can be problematic in the later years of life .
As far as your problem of folliculitis is concerned ,you can apply a topical antibiotic cream like mupirocin over the lesion twice daily .
Wear cotton undergarments so as to decrease sweating and prevent further infection .

Dear Doc,
Thankyou for ur response. As you suggested that circumscision is necessary in my case, i just wanna know that is there any other way in which i can pull my foreskin. I mean i have heard of something called dorsal slit or cant the opening be made a little big via operation instead of removing it full. Also i have heard related to partial circumscision.. Whats that?
I am asking because i have heard that glans after exposure would be very sensitive and would be difficult for me to do my daily activities.
Also let me know the cream which you suggested mupirocin is g6pd safe and not allergic. Or can i continue to use surfaz sn cream for folliculitis and similar rash due to swet and itch.

Hi again

I understand that you are very bothered due to the problem . I will try to answer all your queries .
Dorsal slit is a temporary procedure to relieve the tight ring in patients who suddenly develop a constriction in the phimosed region .So it is basically an emergency procedure and does not serve any long term benefits .
Now regarding a partial Circumscision ,only your operating surgeon can guide you regarding this after proper examination of the degree of phimosis . It is done only in cases of mild to moderate degree of phimosis or adhesions.
Mupirocin is a topical antibiotic cream and is safe to apply in your case .

Thankyou so much for helping out with all my queries. Now my only question remains is that the surgeon for such operation is called what.. I mean any speciality required for it. Are they called plastic surgeons.. Pls let me know
Secondly sometimes i get frequent itch on my scrotum and towards inner groin area. Which cream can i apply over it and since i understand that such itches arise due to moist conditions can i use clocip powder or absorb powder to keep it dry. Will it prevent itch.

Itching and rashes in the groin can commonly occur in fungal infections .
Apply a topical antifungal cream like clotrimazole cream over the affected areas twice daily .
Use clotrimazole powder over the groin folds once or twice a day to prevent recurrent fungal infections
