Question : I have many small pimples on my buttocks and black patch around . There is no pain or itching it is just I doest look very hygienic.

I have viewed the image. You seem to have a fungal infection and postinflammatory hyperpigmentation.
Have you already used any medications?
I suggest you to use a topical anti fungal e.g clotrimazole 1 % cream, twice daily.
