Suggest Treatment For Pineal Cyst And Insomia

Question: I have a 1x1cm pineal cyst my neuro says is not touching anything just floating in fluid & is non issue "incidental finding." I was also recently dx w Mitral Valve Prolapse, PCOS & GAD. I need 100mg of seroquel/night to sleep bc i recently developed insomnia. Could the pineal cyst be to blame for insomnia? My drs say no but I read online cyst can affect melatonin?

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If you would have presented to me as an outpatient with this complaint,I would have told you that - your pineal cyst is not the cause of your insomnia.

Though melatonin is secreted by pineal gland but a pineal cyst,only rarely can increase the melatonin levels,when it is very large in size.1*1 cm floating cyst will not cause it.Even this very fact is theoretical and practically or clinically,it is not being transformed into the cases of insomnia.

So,technically,clinically and practically-there is absolutely no doubt that this pineal cyst is not the cause of your insomnia.

Insomnia has to be evaluated for,whether it is a primary or secondary insomnia?Please observe that you have a good sleep hygiene.Also,you have associated diseases which might be associated with mental problems like depression or anxiety and this actually may be the cause of insomnia.

Thanks for being in follow-up.

You are on seroquel 100 mg and seroquel is not primarily a sleep inducing drug.Rather,it is an antipsychotic drug meant to treat psychosis,bipolar disorder and some part of depression.It induces sleep and that's just a secondary action.So,you are on such high dosage,it means that you are being treated for some of its primary indications and not for secondary effect of sleep induction.

A Psychiatrist's opinion on this issue may be of help.

It is because Dementia is associated with neurotransmitters abnormalities in brain which results in disturbances in the timing of sleep-wake cycle,often characterized by night time wandering and sundowning(increased symptoms at night).
