Suggest Treatment For Poor Concentration

Question: I am 46 white male. I eat a healthy diet ( not perfect) and used to workout regularly but don't have the energy anymore. I can literally lay down on a bench and do a set and then sit up and yawn. But probably what is more concerning for me is my ability to concentrate anymore is disturbing. When i read something I have to read it several times to absorb it and a lot of times when speaking i have to pause to think of the word I'm looking for. Not sure if all that is tied together or I'm a totally wreck??
