Brief Answer:
Infection.

Detailed Answer:
Hi.
Thanks for your query and an elucidate history, noted carefully and understood the problems.

The most probable cause of such a smell that you have to change pillow daily indicates the possibility of an infection in the wounds.
What wonders me is why the is it taking several months for the area to heal.

I would advise you to visit the Dermatologist / Surgeon to see the wound, get the relevant blood tests, secretions from the lesions for gram staining, culture and sensitivity tests before the antibiotics are applied locally or started orally to get appropriate results.

Also discuss about the possibility of the flap-rotation or skin graft surgery for the wound/s to heal fast.
Skin is the best dressing.

Can you please post / attach the clear pictures / photos of the healing area >
YYYY@YYYY > that may help me more to guide you further. Can you please give the details about the problems and the surgery ? When was it done ? Histo-patho (biopsy) reports of the lesion ?

Once the samples are sent for culture and sensitivity, apply liquid Povidone Iodine locally 6 hourly. He may need additional oral antibiotics and metronidazole ( in low dosage for anerobic infection)

I hope this answer helps your husband to have proper diagnosis, local and oral therapy and probably a chance to have a proper surgical intervention if required.

Please feel free to ask further relevant questions or if you feel there is a gap-of-communication.

With regards.
