Suggest Treatment For Radiating Pain In The Arm

Question: My aunt operated for T2N0M0 breast cancer with axillary node dessestion almost 3 months ago and now under 4 TC cycle of chemo. Pain in her arm has still not come down and she complains about the radiating pain. We showed it to a neurologist as well and he suggested to take PENTANERVE NT 400/10 tablet once in a day at bed time.

1. How long it takes for this pain to settle? Will this pain be there life long?
2. I was reading about sentinal node biopsy where only few nodes are removed but unfortuantely we were not having this information during operation and we operated her very quickly after knowing about breast cancer. Her tumor size was 3cm and almost 17 lymp nodes were removed which I feel was uncessary as lymp nodes were not involved. Even doctor didnt tell anything about this procedure during opertaion. Did we do a mistake by removing all her 17 lymp nodes? Is the sentinal node biopsy is practised in XXXXXXX yet?

1.Pain is common in the upper limb after Mastectomy with axillary dissection. I am sorry to say that we cannot tell the time for complete pain relief.Medicines have to be taken till pain subsides.I also add anti inflammatory tablet along with PENTA NERVE NT to my patients with similar complaints.
2.Sentinel lymph node biopsy is practised in XXXXXXX also.Modified radical mastectomy involves removal of affected breast along with axillary lymph nodes. This is the procedure that is done world wide as per Cancer committee guidelines. Your aunt was operated with correct procedure.
Swelling and pain of upper limb are common after this surgery. Treatment is aimed to relieve the symptoms.
Nothing to worry.

My aunt has undergone quadrantectomy with axillary dissestion fir the right breast. It was a clear margin with no node involvlment.

Our surgeon told, no need to perform mastectomy and breast can be preserved.

Do you think mastectomy reduces the further risk of recurrence?

Also, why sentinal node biopsy is practised as standard here in India?

Wide local excision is one option.

If the lesion is small, Wide local excision /Quadrantectomy is practised now.Chances of recurrence also depends on Post operative Radiotherapy /Chemotherapy.If Quadrantectomy doesn't completely remove the tumour margin, recurrence is seen.As the margins are clear in your aunt,nothing to worry.
Sentinel lymph node biopsy is practised in few centres only.If the first node is involved, chances of involvement of other nodes is present. Then,Axillary dissection is done.It varies from centre to centre.But,axillary clearance is world wide accepted procedure in Carcinoma breast.

What is the anti inflammatory tablet she should take? I can check this with medical oncologist when i am taking her for the final chemo on Apr-12th.

After completing the chemo, radiation will start for her. Consdiering her tumor size which was 3cm, do you think we shoukd consider the mastectomy after chemo as a safe option

You can take Aceclofenac with Paracetamol.

Hi
I prescribe Anti inflammatory tablet containing Aceclofenac and paracetamol.
There is no need for mastectomy after chemotherapy.It is indicated only in recurrence of carcinoma.
Please follow the Oncologist advice.
