Question : I've had this white lump on my tongue for 4 days it's raised and is really sore, what should I do? What is it? Is it something bad? How long should I wait before seeing GP or Dentist? I'll attach picture so you can see

I appreciate your concern

Based on the picture you uploaded and the painful white lesion , Aphthous ulceration is the most likely diagnosis.

Vitamin B B1,6 and 12 combination and FOLIC ACID supplementation can be advised for a month.

OINTMENT containing cetalkonium chloride and choline salicyltae can be used locally for the pain associated with the lesion. Antibiotics like Doxycycline course can be prescribed if the lesions persist or worsen despite the above measures. Tobacco chewing and smoking should be strictly avoided.
If the lesions still persist, oral brush biopsy should be considered to rule out the possibility of a malignant lesion in consultation with your doctor.
