It could be anything. Getting a diagnosis like herpes or so is always a visual diagnosis. Please post a photograph and detailed history.

Thanks for the picture; only one is clear enough to understand your problem.
It clearly shows this is folliculitis. meaning infection of the hair follicles.
Continue the antibiotics and local application of the liquid povidone iodine as already advised.
You have not posted the blood sugar reports.
You may please re-post clearer pictures.

I didnt get my blood sugar yet. Here are a few more pics. I noticed these marks on sunday while applying the provodone. I was having trouble keeping dry. Do i apply it and then wash it off with water then dry? Also before i got in shower last night i went number one and it felt like i couldnt finish. Then after i had a small discharge before sleep. Seems like the redness on my scrotum has minimized too since i have maintained dryness. I also sent a pic of the red mark on the penis head. Please let me know what you think. Thank you!!

Seen the additional pictures you have provided.
This is Folliculitis.
The rash on penis and scrotum can be due to folliculitis, showing differently as the skin is different at these parts.
You can apply Povidone Iodine Liquid even after bath, it forms a good antiseptic layer and dries up fast.
Continue Antibiotic for 7 to 14 days and and metronidazole for 5 to 7 days.
What exactly happened when you went for number one ?
The small discharge can be from the glands of urethra or prostate.
The picture of the red mark on penis head is not there, so I could not see it. Please post again.

I added a couple more photos.. When i was taking a leak it felt like i was finishing then it felt like seamen come through.. It happens now and again then later it comes out like a discharge. And sometimes it is just a discharge.. I got another good shot of the rash besides the red marks on my penis. I see a urologist on the 28th this month.. Is there anything you think i should ask or should tell him? Thanks doc. I appreciate your help!!

The skin lesions are just folliculitis and nothing much to worry.
The lesion on the penis is called balanitis and can be part of dermatitis or due to the effect of the urethral discharge.
The attack you had is called a Syncope and some people can have it.
It looks from your history that you are suffering from Prostatitis which is giving you the urethral discharge.
It is great that you will be examined and investigated by the Urologist. Please give him the detailed history about prostitute and how all the problems started- progressed and so on- Undergo all the tests and investigations he would ask you to undergo to make the diagnosis more clear and helping towards cure.
Till then drink plenty of oral fluids and water to keep urinary tract flushed.
All the best for early and permanent cure.
