Welcome back to HCM. What caused the open wound, it looks like you have contact dermatitis, however, I am not sure why you have that wound, did you scratch the rash raw? I would give it time and see if it heals. Please provide events surrounding the rashes appearance, have you been gardening?, occupation?, latex?, animals in the home?, camping recently?, these will assist narrow down for your the probable causes. hello I was told I have prurigo modularity and I scratch and pick the scabs

That makes sense, your scratching and picking is very consistent with it.

Detailed Answer:
Unfortunately this is going to hang around and different treatments work for different people. The cause of prurigo nodularis is not known, it is thought to have some association with autoimmune disorders, some cancers, HIV and many others. The thing is that the scratching is what gets you into trouble with infection setting in, that is why I was concerned about the open lesions. I would use antihistamines to help with the itching and corticosteroid creams should suppress the urge to scratch. If this gets out of hand then it would be time to use immunosuprresants like tacrolimus. I would visit with a dermatologist to assist with treatment. I hope this has been helpful.

ty so much yes u have been battling this skin disease for many years and it's very hard to treat. I have also been diagnosed with lupus I don't know if that contributes to it

The rash associated with lupus is not just on one spot like that. However, the biologics that I mention would also be used in rheumatological diseases like RA, and lupus. So yes, you need to speak to a dermatologist for specialized care and resolution.
