Brief Answer:
Consult ENT and Psychiatrist for your problem

Detailed Answer:
Hi, XXXXXXX ,
Thanks for posting query.
It seems that the depression you are having is main cause of your problem.
I think your depression is not taken seriously or not treated properly.
And I think your old problem for having COPD and Gastritis aggravated your depression.
Another precipitating factor might be Menopause.
And your problem of having chronic ear infection is not properly diagnosed and empirically giving treatment of antibiotic medicine courses.
In your writing query there is impression of having chronic depression as well.
So I advice to consult,

1)ENT specialist and get examined and investigated for proper diagnosis and then treatment accordingly.

2) Psychiatrist for examination and find out the type of depression you have and go for proper treatment and counseling with psychologist.

Attend Menopausal clinic and take proper guidance.

I hope, I answered your query.

If you have further query or clarification, please write back.

Ok and wish you speedy recovery.
