Question: My mom was earlier a tb patient but nw its cured ..its been lik a year tht she is cured from tb .bt nowadaz she has problem with het nose and throat .wen showd doctor he said that she has extra mass inside her nose so he gave injection an all for that and treat was going on but now she had bleeding fromher nose .and at nite wrn evr she goes to sleep she always is coughin constantly asin dometing is itching her in her throt .then it takes long for her to sleep as she keeps on coughin bcs of that iritation .pleace help me with some solution ...
Mom is 51 year old .she is a diabetic aswel.she is curently facing problem with her throat and nose ..she evn consulted an ent nd he said tht her bone inside her nose is nt proper an al bcs f which she had problm to breathe earlier thn the doct strtd gvin her vitamin tablts abd nasal spray ..later he evn gav injection in her nose .bt now she faces constant bleedin at times.please help me with tis .secondly she has constant coughin at night wen she goes to sleep ...and she feels as in smtings itchin her throt and so shes coughin ...day by day shes coughin cobstantly at nite wen she goes to sleep ..plses help me with tis too ....

I have read through your query in detail.
Your mother might have recovered from TB completely but her immunity might still be low. This can cause repeated infections in the nose and in some people fungal infections can occur after TB is cured and this might involve the nose and paranasal air sinuses causing discomfort and bleeding and also appears as a mass.

The other causes of nose bleeding in a person like your mother can be uncontrolled hypertension. However for this her blood pressure must be high and there will be no mass found.

Rarely such a mass that is bleeding can be due to cancer, but since the ENT surgeon has seen her in detail therefore this looks unlikely.

There is also problem with coughing at night with itching in the throat. These can be due to an associated infection in the throat.

In my opinion, she should get a CT scan of PNS (paranasal air sinuses) done and if there is a mass then both plain and contrast scan is required. This will give an idea of what exactly is there within the nose ans sinuses that is causing the mass and bleeding. If it was a simple infection then bleeding should not be there.

Treatment should be as per CT scan findings and focused on the cause of bleeding.

Even if bony septum of the nose is curved then that will also be known after doing the scan.

Is ter any chances that she has tb again ...from the above problems i have mentioned.and any treatment for the coughin which she gets evry day at night while sleeping ...and the ent said tht shes facing problem with the nose bone ..so wt treat ment should be done next ..

Please find my observations below.
1. If treatment is taken properly then the possibility of recurrent infection is less. TB is a disease that can affect any organ from head to feet. Therefore we cannot rule out recurrent infection with TB. However, TB is accompanied with loss of weight and light fever. Please record her body weight every 15 days and see that she does not have weight loss.

2. To know about the mass in nose in detail and also if the symptoms are due to nose septum deviation then that will also be seen on CT scan. This will also tell if there is anything wrong in the sinuses and detailed information will be confirmed.

3. With my experience, I strongly suggest CT scan first to know the size of the problem in the nose and sinuses and then treatment options will be clear.

4. Even if recurrent infection or new infection is suspected then biopsy can be done depending on the CT scan findings.

5. For cough she can take symptomatic treatment like cough syrup and tablet cetrizine 10 mg once at night for 10 days. She should avoid dust and pollution in environment.
